Forms for Unsportsmanlike Conduct and for Exceptional Sportsmanship

Polk County Schools Ejection Procedure

Should it become necessary to eject an athlete or a coach at one of the public high schools in Polk County, the referee is required to send a copy of the FHSAA ejection report to the Polk County District Director of Athletics within 24 hours just as the referee is required to fax this report to the FHSAA.

This copy may be sent either by fax or e-mail:

FAX:          (863) 534-0787
E-Mail:       don.bridges@polk-fl.net

The Polk County School District is striving to eliminate the unsportsmanlike behavior of both athletes and coaches at athletic events.  Players and coaches are both required to pay any fines levied against their school for being ejected from an athletic contest.

Your cooperation in this matter is greatly appreciated.  Together we can make good sportsmanship a part of athletics in Polk County.

The FHSAA form for unsportsmanlike conduct is available to read and/or print out by clicking on the link below.

EXCEPTIONAL SPORTSMANSHIP REPORT

Please report any instance where a coach, student-athlete, team, official, spectator(s), or school in general has done an exemplary job of Sporting the 'Tude at an FHSAA interscholastic athletic contest.  This will help promote and encourage those associated with interscholastic competition to Sport the 'Tude!

The FHSAA form for exceptional sportsmanship is available to read and/or print out by clicking on the link below.  A copy should be sent to the principal and to the FHSAA Office.
